Program Overview
As a  training tool in patient safety, the presentation will cover the complex anatomy of medical errors, classifications of errors and common tools used to mitigate each of them.  We will  discuss  how errors are usually identified and highlight the advantages of  near miss reporting systems.  The ACP/NYACP Near Miss Registry will be highlighted as a patient safety data collection, reporting and quality improvement tool that was developed by Internal Medicine physicians in New York State.  Physicians who submit a confidential report to the risk free Registry will obtain a certificate that verifies their participation in a patient safety activity.

The presentation will help participants to:

  • Report medical errors and near miss events
  • Understand  how well trained professionals commit errors
  • Identify ways to minimize factors that commonly contribute to  medical errors
  • Learn how health care can  utilize “ ultra- safe” principles
